Having been informed through the Privacy Notice (KVKK), I give my explicit consent under Article 6 of the Turkish Personal Data Protection Law No. 6698 for the health data (special category personal data) that I choose to share through the contact and appointment request forms on www.drgokhanpolat.com, by e-mail or via WhatsApp, such as symptoms, diagnoses, treatments, medication and imaging details:
- to be processed by the data controller, Prof. Dr. Gökhan Polat, to assess my request, plan the appointment and examination process and give me preliminary information,
- to be stored, for these purposes only, on the systems of the IT service providers that provide the Site's hosting and e-mail services.
Giving consent is not mandatory. You can contact us and request an appointment without sharing health information; any information needed will be collected separately during the physician's assessment. You may withdraw your consent at any time by writing to [email protected]; withdrawal does not affect the lawfulness of processing carried out before that date.
